About this test
HBeAg is a secretory antigen of the hepatitis B virus and is one of the serological markers used in the diagnosis of hepatitis B virus infection (type B jaundice). This antigen is most commonly found in serum in the presence of the surface antigen HBsAg.
Measuring HBeAg is important for assessing the activity of hepatitis B virus infection. The presence of this antigen in serum indicates active viral replication (multiplication), the presence of viraemia (virus in the blood) and high infectivity. HBeAg is assessed alongside other serological markers of infection, particularly HBsAg and the presence of HBV DNA.
The persistence of HBeAg over a prolonged period may be associated with the disease progressing to a chronic form.
During the course of the disease, HBeAg may disappear and anti-HBe antibodies may appear; this is referred to as seroconversion in the HBe system and signifies the cessation of viral replication. HBeAg may also be negative in the case of infection with one of the mutated forms of the hepatitis B virus, which do not produce this antigen.
Interpretation of results
It should be assessed in conjunction with other serological markers of hepatitis B infection, in particular the presence of the surface antigen HBsAg and HBV DNA.

Symptoms of acute viral hepatitis B
An acute hepatitis B virus infection may be asymptomatic in many of those infected. In some patients, it may progress to a chronic stage, even in the absence of symptoms during the acute phase. Patients with chronic hepatitis B are often unaware of their condition because they have no symptoms. However, they are a source of infection for others. In some patients, chronic hepatitis can progress and lead to cirrhosis and liver cancer.
If acute hepatitis does present with symptoms, these may include:

jaundice (yellowing of the skin and the whites of the eyes),

nausea and vomiting,

abdominal pain (particularly under the right rib cage),

flu-like symptoms,
