A positive HBsAg result indicates infection with the hepatitis B virus, which causes viral infectious inflammation of the liver – viral hepatitis B (type B jaundice). The disease is transmitted through blood and sexual contact, but can also be passed from mother to child during pregnancy or childbirth.

About this test
HBsAg (hepatitis B surface antigen) is a surface antigen of the hepatitis B virus that can be detected in the serum of infected individuals. A positive HBsAg result indicates infection with the hepatitis B virus, which causes viral infectious inflammation of the liver – viral hepatitis B (type B jaundice). The disease is transmitted through blood and sexual contact, but can also be passed from mother to child during pregnancy or childbirth.
HBsAg is present in the serum during acute infection, and if the disease becomes chronic, HBsAg persists in the serum. The presence of HBsAg in the serum for more than 6 months after exposure to the virus suggests a chronic infection or chronic asymptomatic carrier status.
Further tests are required to distinguish between acute hepatitis B and the reactivation of chronic hepatitis B. The presence of Anti HBc IgM antibodies indicates acute hepatitis B. During the replicative phase of the infection (viral replication), HBeAg and positive HBV DNA are also usually present.
Testing for HBsAg, together with testing for total Anti HBc, is recommended as a screening test for hepatitis B virus infection; to confirm or rule out infection, further serological markers of hepatitis B virus infection—in particular HBV DNA and HBeAg—must be tested for if the result is positive. For screening aimed at preventing transmission of the infection via blood transfusion, tissue and organ transplantation, among healthcare workers and pregnant women, HBsAg testing is sufficient; however, in the event of a positive result, testing for other hepatitis B markers should also be carried out.
Notice
Laboratory test results must always be interpreted in the context of the patient’s medical history, clinical condition, and the results of other laboratory and diagnostic tests. Deviations from normal values are not sufficient on their own to establish a diagnosis, and values within the normal range do not always mean that a disease is absent. It is advisable to consult a doctor about the results.
Suitable for
For anyone who wants to find out whether they are infected with the hepatitis B virus, even if they have no symptoms.
Hepatitis B screening (Anti HBs and total Anti HBc must also be tested)
This may be particularly suitable for people with piercings or tattoos, those who have had unprotected sex, those with multiple sexual partners, and those who inject drugs.
In cases of suspected acute hepatitis B, which may present, for example, with jaundice (yellowing of the skin and whites of the eyes), fatigue, nausea, vomiting, abdominal pain (particularly under the right rib cage), dark urine and pale stools.

Symptoms of acute viral hepatitis B
An acute hepatitis B virus infection may be asymptomatic in many of those infected. In some patients, it may progress to a chronic stage, even in the absence of symptoms during the acute phase. Patients with chronic hepatitis B are often unaware of their condition because they have no symptoms. However, they are a source of infection for others. In some patients, chronic hepatitis can progress and lead to cirrhosis and liver cancer.
If acute hepatitis does present with symptoms, these may include:
-
jaundice (yellowing of the skin and the whites of the eyes),
-
fatigue,
-
nausea and vomiting,
-
abdominal pain (particularly under the right rib cage),
-
dark urine and pale stools,
-
flu-like symptoms,
-
joint pain,
-
skin and neurological symptoms.
